This page will provide access to several of the "day-to-day" forms you may need in activities with Black Creek Baptist Church. Each item below will describe the access and print method for the individual forms. If you have questions in regards to the form itself, please contact a representative of the committee or activity the form relates to. | The purpose of this form is for parents/guartdians of youth- (those in grades 6th - 12th) to providde their permission for a youth to participate in the many Youth Ministry activities at Black Creek Baptist Church. This form is updated yearly and parents will need to provide an updated form and information each year. For questions, please contact the secretary's office. |

| Release Form | The purpose of this form is to grant permission to use the images of members taken at church functions and post them on the church website. this form grants the church license to use the person's image free and clear of any copyright claims |
